Work Text

Castiel felt the rush of fiery darkness all around him as he pulled Dean Winchester from Hell, Righteous Man's soul weighted by the agony and memories of damnation.

As he pressed his hand to Dean’s shoulder, the unbreakable mark of his angelic grace seared into human flesh. In that instant, Castiel—a soldier of Heaven—became tied to a single, fragile human.
Dean's scent hit him first, rugged and earthy with a quiet warmth that settled in Castiel's core. At the time, he dismissed it as a side effect of his mission. A bond between an angel and a human? He didn’t question it. But, from that day forward, Castiel’s grace would flicker with the faintest glimmer of peace whenever Dean's soul was near.
Now, he knew something had changed within him, his grace intertwined in ways that neither Heaven nor his own understanding could explain. Dean was his assignment, his charge. Yet, that sense of duty wrapped itself around Castiel’s instincts in ways he hadn’t foreseen—an instinct to protect, to be near him, to shelter this mortal that had somehow stolen pieces of his grace.

The bright lights of the hospital stung Castiel’s eyes as he looked down at Dean, his heart throbbing with an unfamiliar ache. Dean lay unconscious on the narrow bed, his face pale and bruised, lips parted slightly as he struggled for air. He’d survived the showdown with Alastair, but not without damage that cut deeper than the flesh.
Castiel placed two fingers against Dean’s forehead and closed his eyes. Healing energy surged into Dean’s body, but as his grace met with Dean’s internal systems, he froze. Something was terribly wrong.
There was resistance—not just from the bruised flesh or fractured ribs but from something more insidious within Dean’s body, something twisting and turning against him at a microscopic level. Castiel’s eyes snapped open, his gaze tightening on Dean’s pained expression.
“Dean…what is this?” he murmured, voice thick with worry.
Tentatively, he placed his hand over Dean’s chest, his fingers curling in a protective grip as he reached further with his grace. His grace dipped into Dean’s system, mapping out the web of nerves and arteries, following the subtle pulses of life.
That’s when he found it.
An invisible war waged beneath Dean’s skin. His own immune system was attacking itself, like armies sent into battle against their homeland. It wasn’t just the aftermath of torture—it was far worse. Castiel felt a wave of panic rising within him, an emotion he had not felt since he first stepped into Hell and saw Dean’s soul broken and battered.
Dean’s body was attacking his nervous system.
He had read about human illnesses, understood them from the detached knowledge stored in his grace. But this, he realized with horror, was a degenerative disease. Dean’s immune system was attacking the myelin sheaths—the very fibers that allowed his nerves to communicate.
Castiel inhaled sharply, his grip tightening on Dean’s hospital gown. “Multiple sclerosis…” he whispered, barely able to comprehend the words in his own voice. This was not some spell or curse he could simply dispel. It was part of Dean now, like the very blood that flowed in his veins.
He watched Dean’s face, studying the lines of pain that hadn’t faded even in unconsciousness. Castiel let out a slow, shuddering breath. For the first time, he felt the full weight of what it meant to be helpless. This was not an enemy he could banish, a demon he could smite. This was an affliction woven into the very fabric of Dean’s being.
Dean’s eyelids fluttered, his lashes casting shadows on his cheeks. When his eyes finally opened, hazy and unfocused, they sought out Castiel’s presence instinctively, as if he could sense his angelic protector. Castiel felt a surge of warmth in his grace, a feeling he was still learning to recognize. He was bound to this man—connected in ways no celestial order could sever.
“Cas…” Dean’s voice was barely a whisper, hoarse and broken, but it stirred something fierce and primal in Castiel’s grace.
“Dean, my love," Castiel replied softly, his hand still pressed over Dean’s chest. He tried to speak, to explain, but the words seemed hollow in the face of what Dean was about to endure. Instead, he leaned closer, letting his scent wash over Dean, calming him in ways words couldn’t.
“Cas, what’s wrong with me?” Dean’s voice cracked, eyes filling with a mixture of confusion and dread.
Castiel’s heart twisted as he held Dean’s gaze. He knew the truth would devastate him, but lying to Dean wasn’t an option. “Dean…your immune system is attacking itself. It’s called multiple sclerosis. It’s—” he faltered, the helplessness creeping in again. “It’s a disease humans cannot fully heal.”
Dean blinked slowly, trying to process the weight of Castiel’s words. Castiel could see the moment the realization hit—the way Dean’s face slackened, the denial that surged in his eyes only to crumble into despair. Dean’s voice dropped to a whisper, laced with disbelief. “No…no, this can’t be happening. Not after everything else…”
Castiel reached out instinctively, pulling Dean against his chest, wrapping him in an embrace that was both protective and possessive. His instincts roared, demanding that he shield his omega from anything and everything, even the ravages of his own body.
Dean clung to him, his strength waning as the first waves of acceptance and grief hit. He buried his face into Castiel’s shoulder, and for the first time, Castiel felt the hot sting of his own tears. They slipped down his cheeks, silent but unrelenting, mingling with the scent of Dean’s desperation.
“I’m here, sweetheart,” Castiel murmured, his voice a barely contained tremor. “I will be here every step of the way.”

The sterile hum of the hospital filled the silence between them, pressing in on Castiel as he held Dean, feeling the steady but fractured rhythm of his heart against his own chest. Castiel’s hand remained pressed over Dean’s heart, his thumb moving in slow, reassuring circles.
For a moment, neither spoke. Dean seemed almost hollow, his gaze unfocused and glassy as he tried to absorb Castiel’s words.
Multiple sclerosis. Dean had heard the term before—some vague concept, an illness that affected strangers. It wasn’t supposed to be real. Not for him. He was a hunter, his body forged from years of battle, scarred and strengthened in ways most humans couldn’t imagine. This…this weakness couldn’t be his reality.
Dean swallowed, his voice breaking. “How…how bad is it, Cas?”
Castiel hesitated, his heart aching at the question. He wished he could offer false hope, something to lighten the burden Dean was now carrying. But in this moment, honesty was all he had to give.
“It’s different for each person,” Castiel replied softly, his fingers brushing Dean’s arm in soothing strokes. “The disease affects the nervous system, attacking the very connections that allow your body to move and feel. It can lead to fatigue, pain…difficulty with balance, vision, and even simple tasks.”
Dean’s eyes grew glassy, and he blinked hard, as though forcing the tears back. “You mean…this is just going to keep happening?” He looked at his own hands as if they were foreign, tools that might betray him at any moment.
Castiel swallowed, feeling the sharp pang of helplessness twist inside him again. “Yes. And…Dean, there’s no cure. Medicine can help, but…your body will continue to deteriorate over time.”
The silence that followed was almost unbearable. Dean’s face grew tight, muscles straining as he grappled with the weight of Castiel’s words. Castiel watched him, seeing the fight, the refusal to accept this, even as the truth of it began to settle deep within Dean’s mind.
Finally, Dean looked at Castiel, his voice cracking. “So, I’m…just supposed to sit around and watch myself fall apart? Watch my body turn on me?” A tremor passed through his hands, and Castiel gently reached out, wrapping his own larger hand over Dean’s to steady him.
“No,” Castiel replied, his voice firm with conviction. “You don’t face this alone, Dean. I am here, and I will carry you through every step, every moment. You are my omega—my mate. This is my duty.”
Dean’s breath hitched, and he let out a broken laugh, one that held no humor. “Your duty…to babysit me while I lose myself.”
“Not a duty,” Castiel corrected, his tone gentle but unyielding. “A choice. One I make willingly. Every moment, I choose you, my love.”
Dean’s body shuddered, his shoulders rising and falling as the weight of it all broke over him. The strong front he’d tried to keep up collapsed in an instant, and he buried his face in Castiel’s shoulder, a muffled sob tearing from his throat. All the anger, grief, and helplessness poured out in waves, and Castiel could feel the ache of it vibrating through his very soul.
Castiel tightened his hold, wrapping his arms around Dean’s trembling form, his hand instinctively moving to the back of Dean’s head, pressing his nose into Dean’s hair. He let out a soft hum, a sound meant to comfort, as his grace radiated warmth and calm around Dean’s frayed spirit.
“Let it out, my wounded human,” he whispered, his own tears slipping down his cheeks, each one mingling into Dean’s hair. “You don’t have to be strong right now.”
They stayed that way for a long time, Castiel rocking gently, his fingers tracing slow patterns over Dean’s back. Dean’s cries gradually softened, the harsh sobs fading into quiet gasps until he lay still, utterly spent. Cas pressed his forehead to the top of Dean’s head, allowing his alpha scent to saturate the air, wrapping around them like a blanket.
As the silence settled, Dean stirred, his voice barely a whisper. “I…I can’t do this alone, Cas.”
Castiel’s heart ached at the quiet admission. He pulled back, just enough to look into Dean’s face, his hand rising to cup his cheek. “You won’t have to, my love," he promised, his voice low and steady. “I will be by your side, always. Whatever happens, whatever this illness takes from you, it will never take away the bond we share.”
Dean leaned into Castiel’s hand, his eyes slipping shut, exhausted. He could feel the comforting heat of Cas’s grace within him, like a soft glow in the dark—a warmth that soothed the aches within him. And though his body felt like it was slipping away, his mind, for the moment, found a sliver of peace.
But that peace was short-lived.
As Cas brushed his thumb along Dean’s cheek, Dean’s vision blurred, and he blinked hard, trying to focus. It was as though a curtain had dropped over his eyes, dulling the room into shades of shadow and light.
“Cas…” Dean’s voice was tinged with panic as his vision darkened, the edges of the world fading into nothing. “Cas, I…I can’t see…”
Cas’s heart thundered, and he placed a gentle but steady hand over Dean’s chest, his grace pulsing with determination. “Stay calm, Dean. I’m here.”
Dean’s hands gripped the edge of the hospital bed, muscles tensing as he tried to steady himself. A shiver ran through him, and he felt a strange numbness spread from his hands to his arms, crawling up to his shoulders. It was as though his very body was slipping beyond his control.
Without warning, his legs went slack, and he collapsed back against the bed, his strength vanishing in an instant. His breath came in shallow gasps, and he looked up at Castiel, fear flashing in his eyes. “Cas, what…what’s happening to me?”
Castiel leaned forward, pressing his palm to Dean’s chest, feeling the erratic beat of his heart beneath his hand. He could sense Dean’s body turning on itself, nerves misfiring, signals crossing into chaos. He had read of the symptoms of multiple sclerosis but had never felt them as vividly as he did now, watching his omega struggle against his own body.
“I…I don’t know how to stop this, my beloved,” Cas admitted, voice thick with anguish. “But I'm here. I'll hold you and I'll try to reduce your pain."
Dean closed his eyes, letting Cas’s warmth envelop him, allowing himself to lean into the strength of the angel’s embrace. “Just…don’t leave me,” he murmured, his voice a faint echo of his usual confidence.
“I'll never leave you, sweetheart." Castiel replied firmly, his voice laced with a promise that cut through his own helplessness. He gently lowered Dean onto the bed, arranging his limbs with reverence, his every touch infused with the devotion of an alpha protecting his omega.
And as Dean finally slipped into a restless sleep, Castiel stayed by his side, his fingers brushing Dean’s temple as if to shield him from the pain that awaited in his waking hours. He sat vigil over him, his heart heavy with worry and a fierce determination, feeling for the first time the raw, unyielding strength of love.

The days that followed passed in a slow, painful haze. Dean found himself confined to the hospital bed, his once-strong body betraying him in ways he never could have imagined. He could feel it—this enemy within him, gnawing away at his strength, bit by bit. The smallest tasks felt monumental now, as if his body were made of lead. Every inch of him ached, from his limbs to the dull, throbbing pain that had settled in his spine.
Castiel was there at every moment, a silent, protective presence. The angel barely left Dean’s side, only moving to fetch water, food, or whatever the nurses said Dean needed. Each time Dean’s eyes opened, Castiel was there, his gaze steady, calm, and unwavering. When Dean’s hand trembled as he reached for a glass of water, it was Cas’s hand that steadied him, that brought the glass to his lips, guiding him as gently as though Dean were made of glass.
It was early morning, the first light creeping through the window, casting faint shadows over the sterile walls, when the reality of it all came crashing down on Dean.
His hand shook as he tried to lift a spoon, his fingers refusing to cooperate. He gritted his teeth, his jaw clenched as he forced his muscles to obey, but the spoon slipped from his grasp, clattering onto the tray with a dull thud. Dean’s shoulders slumped, his eyes fixed on the spoon as if it were the final blow in a fight he hadn’t signed up for.
“Dean…” Cas’s voice was soft, tinged with concern. He reached out, but Dean pulled back, his frustration bubbling over.
“I can’t even hold a damn spoon, Cas!” Dean’s voice cracked, raw and laced with anger. He clenched his fists, the muscles in his forearms spasming from the effort. “I can’t…do anything. My body won’t listen. I’m trapped in here.” He jabbed a finger into his chest, his frustration spilling over into despair.
Cas watched him, his heart aching as he took in the rage and pain etched into every line of Dean’s face. He wanted to pull him close, to wrap him in his wings and shield him from the cruelty of his own body. But Dean needed more than just comfort—he needed someone to stand beside him in his suffering.
Gently, Cas leaned forward, taking Dean’s hand in his own, his grip firm and grounding. “Your strength isn’t in what you can or cannot do with your hands, my love. It’s here,” he said, his fingers brushing over Dean’s chest, where his heart beat faintly beneath layers of grief and anger. “You are stronger than this illness.”
Dean’s gaze softened for a moment, his lips pressing together as he tried to steady his breathing. But then, as if in defiance, his legs began to tremble, a spasm overtaking his muscles. He sucked in a sharp breath, pain shooting up his spine, spreading like wildfire. Cas’s hand moved instinctively to steady him, his grace pulsing through his touch, a warmth that wrapped around Dean like a balm.
“It’s okay,” Cas whispered, his voice steady even as Dean’s body fought against itself. “I’m here. Let me help you.”
Dean didn’t have the strength to argue. He closed his eyes, allowing Cas to take control, his angel’s touch anchoring him. He felt Cas’s hand rest over his heart, the pulse of his grace reaching into him, soothing the ache that had settled deep within his bones. Cas’s scent surrounded him, calming the jagged edges of his mind, grounding him when his body felt like a prison.
The hours stretched into days, and Dean’s symptoms grew worse. The fatigue was unrelenting, pressing down on him with a weight that felt insurmountable. His legs grew weaker, to the point that he could barely stand without Cas’s support. Cas would be there each time, his arms strong and unwavering, his eyes filled with a quiet determination that somehow gave Dean the strength to keep going.
One evening, as the sun dipped below the horizon, Dean sat slumped in his bed, his body aching, his spirit frayed. Cas was there, as always, his presence a constant source of comfort. Dean’s hand trembled as he reached for the glass of water Cas held out to him, his fingers curling around it with difficulty. As he brought it to his lips, he felt a shiver run through him, and the glass slipped from his hand, spilling water across the sheets.
Dean’s face crumpled, and he turned away, shame burning in his chest. “I can’t even drink a glass of water,” he muttered, his voice choked with frustration.
Without a word, Cas reached forward, gently cupping Dean’s cheek, guiding his face back so their eyes met. “You don’t have to do this alone, Dean,” he murmured, his voice a gentle caress. “Let me help you.”
Dean’s shoulders slumped, and he gave a small nod, too tired to argue. Cas took the glass, holding it carefully to Dean’s lips, his other hand resting on Dean’s shoulder, steady and reassuring. Dean drank, his eyes slipping shut as he let Cas’s strength carry him through the simple, humbling act.
When the glass was empty, Cas set it aside, his hand lingering on Dean’s shoulder. He leaned forward, his forehead pressing against Dean’s, a gesture filled with a tenderness that Dean hadn’t realized he needed.
“You are mine. And nothing—not even this illness—will change that.” Cas whispered, his voice a soft, steady anchor.
Dean’s breath hitched, and he closed his eyes, letting the words sink into him, soothing the raw edges of his soul. He felt Cas’s hand move to the back of his head, his fingers threading through his hair, the simple touch grounding him in a way that nothing else could.
In that moment, Dean felt the weight of his illness ease, just a fraction, replaced by the warmth of Cas’s unwavering love. He leaned into Cas’s touch, allowing himself to be vulnerable, to let go of the walls he’d built around his heart.
“Cas…” he murmured, his voice barely a whisper. “I…I don’t know how to do this.”
“You don’t have to know,” Cas replied softly, his thumb tracing gentle circles against Dean’s scalp. “You just have to let me be here with you. Let me carry you, my love. Let me be the strength you need.”
Dean’s eyes stung with unshed tears, and he let out a shaky breath, nodding. He didn’t have the strength to fight, but he didn’t need to—not with Cas by his side, holding him up when he felt like he was falling.

Dean lay in his bed, staring up at the dim, unforgiving ceiling of his hospital room. He could hear the quiet hum of the machinery nearby, the soft footsteps in the hall outside, and the steady, soothing breaths of Cas beside him. But even with Cas’s presence wrapping around him, a heaviness settled in his chest, a weight that he couldn’t shake.
For days, he had fought against the anger, the resentment that simmered inside him. But now, as his own hands failed him and his legs felt like lead, all that he’d been holding back clawed its way to the surface, relentless and raw.
With a shaky hand, Dean reached for the glass of water on the bedside table, the action simple yet loaded with the exhaustion of his condition. His fingers trembled, his grip slipping, and the glass toppled, water spilling across the floor. Dean’s heart sank, the familiar frustration bubbling over.
“Damn it,” he muttered, his voice low, thick with suppressed anger. He clenched his fists, feeling the uselessness of his own hands, his own body. “I can’t even do something as simple as drink a glass of water.”
Cas, sitting by his side as always, quietly reached out, his hand resting on Dean’s shoulder, his touch as steady as it was gentle. “Dean, it’s okay. Let me—”
“No!” Dean’s voice was sharper than he intended, cutting through the quiet, startling even himself. He gritted his teeth, anger and despair flashing in his eyes. “It’s not okay, Cas. Nothing about this is okay.”
Cas withdrew his hand, his gaze steady and patient as he allowed Dean’s anger to fill the room. He didn’t interrupt, didn’t try to soothe it away. Instead, he waited, letting Dean’s pain spill out without judgment or reproach.
Dean’s breath hitched, his vision blurring, and he shook his head, words tumbling out as he struggled to contain his emotions. “I used to be strong. I could fight, I could protect people. Now…now I can’t even hold a glass. I can’t…do anything. I’m just…stuck here, useless.”
Cas’s chest tightened, each word sinking into him like a stone. He could see the raw pain, the fear, the sense of betrayal Dean felt toward his own body. And he knew that no words could truly take that pain away. But he would stay, he would be here through every wave of anger and sorrow.
Without a word, Cas shifted closer, his hands moving with deliberate gentleness. He reached for Dean, pulling him into his arms, letting Dean’s head rest against his shoulder. Dean didn’t resist. His breaths were ragged, shallow, as he clung to Cas, his fists gripping the fabric of Cas’s trench coat.
“I hate this, Cas,” Dean whispered, his voice breaking. “I hate feeling like this. I hate feeling…weak. I’m supposed to be strong. I’m supposed to be able to protect the people I care about.”
“You are still strong, Dean,” Cas murmured, his fingers gently brushing through Dean’s hair. “Strength is not measured by what you can hold or carry. It’s in your heart, in the way you keep going even when it feels impossible.”
Dean’s shoulders trembled, his anger melting into something deeper, something that hurt even more. He buried his face into Cas’s shoulder, his voice barely a whisper. “I don’t want to be a burden, Cas.”
Cas’s grip tightened, his voice soft but fierce. “You could never be a burden to me, Dean. You are my mate. I am here because I want to be, because you are worth every moment of my care, my love.”
The words sank into Dean, wrapping around the jagged edges of his heart, filling the empty spaces with a warmth he hadn’t realized he was missing. He felt Cas’s fingers press lightly against the back of his neck, a soothing, grounding touch that reminded him of their bond, of the invisible thread that connected them.
Slowly, Dean’s breathing evened out, his anger giving way to exhaustion. Cas’s scent surrounded him, warm and calming, a subtle, protective aura that grounded him. And as he leaned into Cas’s embrace, he felt, for the first time, the raw edges of his own pain begin to soften, if only a little.
They stayed like that for a long time, Cas holding him, neither speaking as they let the silence settle around them. Dean closed his eyes, feeling the steady thrum of Cas’s heartbeat beneath his cheek, the warmth of his arms a shield against the darkness of his own thoughts.
“I’m here, my Righeous Man,” Cas murmured, his voice barely above a whisper. “I’ll be here every step of the way. Whatever this illness takes, it will never take away what we have.”
Dean felt the burn of tears in his eyes, the emotions too raw, too powerful to contain. He let out a quiet, broken sound, and Cas’s arms tightened around him, holding him close, letting him fall apart in the safety of his embrace.
And for the first time, Dean allowed himself to let go, to lean on Cas fully, his heart open and vulnerable. He let the tears fall, his pain pouring out in waves, and Cas held him, his own tears slipping down as he whispered soft words of comfort.
“You are not alone,” Cas murmured, pressing a soft kiss to the top of Dean’s head. “I am here, and I always will be.”
The quiet promise settled over Dean, filling him with a sense of peace he hadn’t felt in a long time. He clung to Cas, his breathing evening out, the weight of his illness still there but no longer unbearable. With Cas by his side, he knew he could face whatever came next.
And in that quiet, fragile moment, Dean knew that no matter how much his body betrayed him, he would never have to face it alone.

Days turned into weeks, and Dean’s condition steadily worsened, robbing him of small freedoms that once seemed so effortless. His hands no longer obeyed him, often clenching into stiff, painful fists he could barely unfurl. His legs grew weaker until even standing required Cas’s strong arms lifting and supporting him. The tremors that occasionally wracked his body were relentless, each one a reminder of the war his own body waged against him.
Castiel never faltered. He was always there, his steady hands guiding, lifting, supporting, and soothing. As the disease took more and more of Dean’s independence, Cas took on the role of his silent strength, tending to him in ways that were both practical and deeply personal. Every small gesture was an expression of love, of a bond that had grown into something unshakable.
One afternoon, as a light rain pattered against the hospital window, Cas sat beside Dean’s bed, a plate of food on a tray in front of him. He lifted a spoonful, bringing it toward Dean’s lips with a gentle patience that had become his nature. Dean opened his mouth obediently, letting Cas feed him, his pride bruised but tempered by the tender look in Cas’s eyes.
“You don’t have to do this,” Dean murmured, his voice soft and hoarse. “I can…figure it out.”
Cas smiled, the corners of his eyes crinkling with a warmth that chased away the shadows lingering in Dean’s heart. “Dean,” he replied gently, “I want to do this. I want to take care of you. There is nowhere else I would rather be.”
Dean’s eyes dropped, a flush of emotion washing over his cheeks. He wanted to argue, to insist he could handle himself, but the truth settled heavily within him—he couldn’t do this alone. And, somehow, that was becoming okay, knowing Cas was here with him.
Cas brought another spoonful to Dean’s lips, his hand steady and sure. Dean ate in silence, feeling the warmth of Cas’s unwavering attention settle over him, a balm to his fractured spirit. As Cas fed him, he murmured quiet reassurances, his voice soft and soothing, each word wrapping around Dean like a promise.
When the plate was empty, Cas set it aside, his fingers brushing over Dean’s cheek, tracing gentle patterns that calmed Dean’s mind. Dean leaned into the touch, his eyes slipping shut as he let himself be vulnerable, safe in the presence of his alpha.

The weeks continued, and as Dean’s body declined, Cas became more protective, more devoted, his instincts as an alpha fully engaged. The once unflappable angel now flinched at even the slightest sign of discomfort on Dean’s face, rushing to his side, his hands gentle but firm, guiding Dean through each wave of pain and exhaustion.
Dean soon found himself relying on a wheelchair, his legs unable to bear his weight for more than a few minutes. Cas took charge, lifting him from bed to chair, moving him with a care that made each transition feel effortless. Dean marveled at the strength in Cas’s arms, the way he could lift him as though he were weightless, a reminder of the angel’s immense power tempered by his tenderness.
One afternoon, Dean’s chest tightened, a sudden wave of panic flooding his senses as he struggled to breathe. His head tilted back, gasping for air, his vision blurring with a familiar, frightening darkness.
Cas was there in an instant, his hands on Dean’s shoulders, guiding him to a more upright position, his voice calm and reassuring. “Dean, look at me,” he whispered, his eyes locking onto Dean’s. “Breathe with me slowly, my love."
Dean’s breaths were ragged, each one catching in his throat, but Cas’s voice, low and soothing, became his anchor. Cas placed a gentle hand over Dean’s chest, his warmth seeping into him, his grace spreading through Dean’s body like a balm.
With each breath, Dean felt the tightness in his chest ease, his vision clearing as he focused on Cas’s face, on the steady strength in his gaze. Finally, his breathing returned to normal, the panic subsiding, leaving him feeling drained but grateful.
Cas’s hand lingered over Dean’s heart, his thumb tracing slow circles. “You’re safe,” he murmured, his voice a quiet promise. “I’m here, Dean. I’ll always be here.”
A weak smile flickered on Dean’s lips, his hand reaching up to cover Cas’s, fingers trembling as he gripped him. “I love you, Cas."
Cas’s gaze softened, a fierce protectiveness shining in his eyes. “I love you too, Dean.” he replied, his voice laced with an unbreakable determination. He leaned down, pressing a gentle kiss to Dean’s forehead, his lips lingering in a promise of love and devotion.

As the days passed, Cas became even more vigilant, anticipating Dean’s needs before he even had to ask. Each morning, he would help Dean wash and dress, his touch reverent, as though he were handling something sacred. He would lift Dean into the wheelchair with practiced ease, his arms strong and sure, always careful, always gentle.
The days blurred into a routine, a rhythm that brought a quiet peace to the chaos of Dean’s illness. Each time Dean’s body betrayed him, Cas was there, his presence steady and unwavering, his hands guiding, supporting, and holding him together when he felt like he was falling apart.
One evening, as Cas lifted Dean from his wheelchair to his bed, Dean looked up at him, his gaze soft and filled with a vulnerability he rarely allowed himself to show. “Cas,” he murmured, his voice quiet but steady. "You’ve been everything to me. I don’t know how to thank you.”
Cas’s expression softened, his hands gently lowering Dean onto the bed before sitting beside him. “There is no need for thanks, Dean." He reached out, his hand resting over Dean’s, his fingers curling around him with a gentle possessiveness. “You are my omega. My love for you goes beyond words.”
Dean’s chest tightened, his throat thick with emotion as he looked up at Cas, the weight of the angel’s devotion settling over him, filling the hollow spaces within him. “I never thought…I’d have something like this,” he whispered, his voice rough. “But you…you showed me I don’t have to be alone.”
Cas’s eyes softened, and he reached up, his fingers brushing through Dean’s hair, a gentle, grounding touch. “You will never be alone, my love.” he murmured. “I'll always be here for you. And I will be by your side through every struggle, every pain."
A quiet peace settled over Dean, and he closed his eyes, allowing himself to sink into Cas’s embrace, to let go of the fear and pain, if only for a moment. Cas’s arms wrapped around him, holding him close, a silent promise that filled him with a sense of belonging he hadn’t known he needed.
As they sat together, wrapped in each other’s warmth, Dean felt, for the first time, a sense of acceptance. His body may have betrayed him, but Cas’s love was unwavering, a constant source of strength in a world that felt increasingly fragile.
In that quiet, gentle embrace, Dean realized that Cas was more than just his protector. He was his anchor, his sanctuary, and the embodiment of a love that would endure, no matter what the future held.

The changes were subtle at first—a hand that trembled too much to hold a glass, a foot that wouldn’t lift without dragging. But over the weeks, the symptoms began to cascade, each one adding to the relentless toll on Dean’s body. By now, every day was a test of endurance. Each moment, each movement, required effort that left him exhausted.
One evening, Cas carefully sat next to Dean's bed, massaging Dean's legs and arms in a way that was enhanced by his grace. But then Dean felt his muscles seize up, a spasm radiating from his legs up through his back, a pain so sharp it left him gasping. Cas was there, his arms wrapping around Dean’s torso, steadying him, his voice calm and grounding.
“Breathe with me, my love.” Cas whispered, his hand pressing gently against Dean’s chest, feeling the rapid thud of his heart. “Focus on my voice. Let the pain pass.”
Dean nodded, his breaths shallow and quick as he leaned into Cas’s embrace, drawing strength from his calm presence. He felt the warmth of Cas’s hand, the steady pulse of his fingers tracing gentle circles over his chest, and the pain began to subside, melting under the weight of Cas’s grace.
When the spasm passed, Cas guided Dean onto the bed, adjusting the pillows behind him, his movements careful and practiced. Dean lay back, his head sinking into the soft fabric, his body exhausted, his spirit worn thin. Cas settled beside him, his hand resting on Dean’s shoulder, grounding him in the here and now.
Dean turned his head, meeting Cas’s gaze, a faint smile tugging at the corners of his lips. “Guess I’m giving you a workout, huh?”
Cas’s expression softened, a tender smile breaking through the sadness in his eyes. “I don’t mind. You are worth every effort, sweetheart.” He reached out, his fingers brushing through Dean’s hair in slow, soothing strokes. “I am honored to care for you.”
Dean’s smile faltered, his throat tightening. There was a part of him that still fought against the helplessness, that resented his own body for its betrayal. But in moments like this, with Cas by his side, he found a strange kind of peace—a comfort that went beyond words.
“Cas…” Dean’s voice was soft, filled with a vulnerability he rarely let show. “I don’t know how you put up with me.”
Cas tilted his head, his gaze unwavering. “Dean, you are my mate, my omega. To care for you is not a burden; it is my purpose. I am here because I love you, and nothing—not even this illness—will ever change that.”
Dean’s heart ached with a mix of gratitude and sorrow. He closed his eyes, letting Cas’s words wash over him, each one a balm to his wounded spirit. He felt Cas’s hand slide down, resting over his chest, his fingers tracing gentle patterns, grounding him in the here and now.
They lay together in the quiet, the only sound the soft hum of Cas’s grace, a presence that wrapped around Dean, soothing the frayed edges of his mind. He knew his body was failing him, that each day was a battle against an unseen enemy. But in Cas’s arms, he felt safe, anchored to something greater than himself.
The hours passed in a peaceful silence, Cas’s hands never leaving him, his touch a constant reminder of their bond. Dean drifted in and out of sleep, his body exhausted, his mind slipping into a fragile calm. Each time he stirred, Cas was there, his gaze steady, his presence unwavering.

As the weeks turned into months, Dean’s body grew weaker, his independence slipping away with each passing day. Simple tasks—eating, drinking, even shifting his weight—required Cas’s assistance. And yet, through every moment, every struggle, Cas remained by his side, a constant source of strength and comfort.
One night, as Cas helped Dean settle into bed, Dean looked up at him, his gaze filled with a quiet determination. “Cas…can you stay with me tonight?”
Cas’s eyes softened, a faint smile crossing his lips. “Of course, my beloved. I am here for as long as you need me.”
Cas carefully climbed into bed beside him, his arms wrapping around Dean’s frail form, holding him close. Dean relaxed into the embrace, feeling the warmth of Cas’s grace seep into his skin, calming the aches that lingered in his body. Cas’s wings unfurled, wrapping around them both, a shield against the world, a sanctuary built of feathers and light.
In the safety of Cas’s arms, Dean felt his mind drift, a sense of peace settling over him that was as rare as it was precious. Cas’s hand rested on his chest, his thumb tracing slow, gentle circles, a grounding touch that kept Dean anchored.
As they lay together, Dean felt a surge of emotion rise within him—a mix of love, gratitude, and a fierce longing to hold on to this moment, to the connection they shared. He turned his head, meeting Cas’s gaze, his voice barely above a whisper. “I love you so much, Cas. I'm proud to be your omega."
Cas’s expression softened, his fingers brushing through Dean’s hair. "I love you too, Dean. And I'm proud to be your alpha."
Dean closed his eyes, his heart aching with the depth of his love for Cas. He leaned into the angel’s embrace, letting the warmth of Cas’s grace fill him, a quiet promise that, no matter how much his body betrayed him, he would never be alone.
Cas’s arms tightened around him, his lips pressing a gentle kiss to Dean’s forehead, a silent vow of love and devotion. And in that quiet, tender moment, Dean found a peace he hadn’t known was possible, a sanctuary built from the strength of Cas’s love.
As the night deepened, Cas held him close, his wings a soft, comforting weight around them both. And in the darkness, wrapped in Cas’s embrace, Dean felt, for the first time in a long time, that he was truly, undeniably safe.

The soft light of dawn began to filter through the curtains, casting gentle hues across the room as Cas sat quietly beside Dean’s bed. He watched the rise and fall of Dean’s chest, each breath a reminder of their journey, of the challenges they had faced and the bond they had forged. The world was still, and in that silence, Cas felt the fullness of his love for Dean, a love that had grown deeper with each hardship, each moment of shared vulnerability.
Dean stirred, his eyes fluttering open, and he looked up at Cas, a faint, tired smile crossing his lips. Though his body had grown frailer, his spirit shone through, an indomitable strength that Cas cherished with every fiber of his being.
“Morning, Cas,” Dean murmured, his voice soft, a quiet warmth in his gaze as he looked at his angel.
“Good morning, sweetheart,” Cas replied, his hand reaching out to gently cup Dean’s cheek, his thumb brushing over the faint stubble there. “How are you feeling?”
Dean gave a small shrug, his smile fading a little. “Tired. But…better, with you here.” His hand reached up, trembling as he took Cas’s hand, holding it with a quiet, reverent grip. “I don’t know what I’d do without you, Cas.”
Cas’s heart tightened, and he leaned closer, his voice barely above a whisper. “You’ll never have to find out. I am here, Dean, always. You're my everything and nothing will ever change that.”
Dean’s eyes softened, and he closed his eyes, leaning into Cas’s touch, his body relaxing as he felt the warmth of Cas’s grace wrap around him, soothing his aches, filling him with a sense of peace that he hadn’t known before Cas had entered his life. In Cas’s arms, he felt safe, whole, loved beyond measure.
They sat together in the quiet, the only sound the gentle hum of Cas’s grace, a presence that wrapped around Dean like a comforting blanket. Cas’s wings unfurled, creating a cocoon of warmth and light around them, a shield against the world. Dean felt himself relax, his breathing evening out as he let go of the weight he carried, trusting in Cas’s strength.
“Cas,” Dean whispered, his voice barely audible, “Thank you. For everything. I…I wouldn’t have made it without you.”
Cas’s heart swelled with emotion, and he leaned down, pressing a gentle kiss to Dean’s forehead, a quiet, tender vow of his unbreakable love. “It is I who should thank you, Dean. For allowing me to be by your side. For teaching me what it means to love.”
Dean’s eyes grew misty, and he looked up at Cas, his voice thick with emotion. “You’re the best thing that’s ever happened to me, Cas. I mean that.”
Cas felt his own tears slip down his cheeks, and he pressed his forehead against Dean’s, his voice a barely-contained tremor as he whispered, “And you are my heart, Dean. My light, my love. No matter what comes, I will be with you, always.”
They stayed like that for a long time, wrapped in each other’s presence, a quiet strength binding them together, unyielding and eternal. Cas held him close, his arms a sanctuary, his wings a shield, and in that moment, they needed nothing else. They had each other, a love that transcended pain, illness, and even the boundaries of Heaven and Earth.
As the first rays of sunlight bathed them in a gentle glow, Cas knew that no matter what trials lay ahead, they would face them together. He had found his purpose, his mate, and his reason to love. And in that quiet, tender embrace, Cas and Dean became more than just an angel and his omega—they became two souls bound beyond the flesh, inseparable, eternal.
